Yep, Its The my Mutant mod! This Mod is a Combination of The Piggsy mod's Texture and the Bigfoot Mod's Model.

This Mod will change one of Your Homies or any ped of your choice into an Ugly Beast!

After you add files to a .img file with IMGtool you should always press the "commands" tab then pick rebuild archive, it might take a while tough.
